People registered with practices in our Wakefield Health Alliance area can now access health and wellbeing coaching; a brand new service available to anyone looking to make positive, lasting change to their health.

Busy lifestyles. Long-term health conditions. Difficulties and issues at home. They all affect how our bodies work and feel and are a common reason why people visit their GP. These complex issues can often change how we eat, sleep, feel and stay active; the main building blocks that keep us well.
Pills and medicines can only help so far which is why you might find your healthcare professional recommends you try health and wellbeing coaching, a dynamic, proven approach for lifestyle-related health problems.
A health and wellbeing coach has time to listen and work with you about the things that matter to you. Not just in treatment but in life. At each session, you’ll talk about where your health and wellbeing is at, set goals for yourself and be guided towards activities, tools, people and things in your life that may help if you need them. Led entirely by you, with your coach right behind you, motivating and encouraging you. Every step of the way.
Our experienced health coaches come from a range of backgrounds and life experiences. Nutrition, physical activity, sleep and stress management are just some of the areas they can think through and work with you on to find the most ideal ways to make positive, lasting change in your life. Think of them as your personal wellbeing spotters!
Speak to a healthcare professional at your GP practice if you feel you would benefit from this service and want to make changes that will help get you on the right track with our support.
Ask them to refer you to the Health & Wellbing Coaching Team.

We welcome referrals for patients with long-term lifestyle related health conditions such as Type 2 diabetes, obesity, high blood pressure) who, with support, are willing and eager to make sustainable health behaviour change to improve their own wellbeing.
We particularly welcome referrals from those with long-standing or newly-diagnosed metabolic imbalance too.
