Link workers deliver holistic health management or ‘non-medical prescriptions’ to those who can benefit from them. One day they might be putting a person in touch with a debt charity, the next they could be addressing their housing needs alongside a housing officer.
Each primary care network has a dedicated social prescribing link worker.
